Cauda Equina Syndrome: A Surgical Emergency
Cauda Equina occurs when the nerve roots in the lumbar spine are compressed, cutting off sensation and movement. The nerve roots that control the function of the bladder and bowel are especial vulnerable to damage. Cauda equina syndrome is one of the only surgical emergencies in the spine and should be taken very seriously. Watch this video to learn more about cauda equina syndrome, what it is, what are the symptoms, and what to do if you have them.

HELLO DR BRIAN. I HAVE JUST SEEN THIS VIDEO, AND YOU ARE BRILLIANT. PLENTY OF TLC, WHILST AT THE SAME TIME, TELLING IT AS IT IS. HAVING SPENT MOST OF MY LIFE IN HOSPITAL I APPLAUD " SAYING IT AS IT IS" ! I MAY HAVE FOUND YOU TOO LATE, BUT IS IT OK IF I ASK YOU SOME QUESTIONS? I WAS BORN WITH A " FRIDAY AFTERNOON JOB...(WHEN GOD LEFT THE ANGELS TO FINISH MAKING ME !) SORRY....TO BE SERIOUS. I WAS BORN IN 1964 WITH HUGE CLOACAL EXTROPHY. WAS RUSHED TO GREAT ORMOND STREET AS SOON AS I WAS BORN, AND STAYED THERE ALMOST UNTIL MY 1ST BIRTHDAY. LOTS OF INSIDES FROM WAIST DOWN WERE EITHER MISSING OR SO TANGLED THEY WERE AROPHIED NO BLADDER,BOWEL,WOMB, URETHRA, VAGINA. ETC. OOPS, FORGOT, JUST ONE KIDNEY, ( WHICH IS HOVERING BETWEEN 3RD AND 5TH STAGES. I ALWAYS THOUGHT IT WOULD BE MY KIDNEY THAT WOULD SEE ME OUT, (AS NO OPTION OF TRANSPLANT) , I HAVE A MITROFANOFF POUCH, WHICH WAS A BAD MOVE. SO, ALONG WITH ALL THAT ON GOING SURGERY, I HAVE HAD A STROKE, 2 DVT, APPENDICITIS, NEWLY DIAGNOSED OSTEOPOROSIS, REPEATED BOWEL SURGERY AND GYNAECOLOGICAL OPERATIONS , I KNOW IVE MISSED SOME THINGS OUT, BUT THESE WERE KIND OF THE BIGGER THINGS ! I HAVE BEEN EXTREME PERIPHERAL NEUROPATHY, AND LOSS OF FUNCTION IN BOTH HANDS. FINALLY AFTER 4 LONG YEARS DURING WHICH I KEPT ON AND ON SAYING THAT I THINK THERE IS MORE TO IT, SOMEONE LISTENED. ! I WAS SOOO GRATEFUL, AND HAVE HAD REPEATED NERVE CONDUCTION TESTS, CT, AND MRI, WHICH GAVE THE FOLLOWING DIAGNOSIS. I WILL TRY AND WRITE THE STAGE I AM AT NOW.....I WOULD VALUE HUGELY YOUR THOUGHTS, WHILST ALSO HELPING OTHERS GOING THROUGH SIMILAR ... ALTHOUGH I AM S... SCARED. IF I CANT COPY WHAT IT SAYS, I MAY JUST TRY SCANNING THE LATEST LETTER..? ..? I HAVE MY BIG MEETING WITH A SURGEON FROM SOUTHAMPTON HOSPITAL ON WEDNESDAY.....IVE LEFT IT VERY LATE ( PLUS THERE IS A HUGE TIME DIFFERENCE) BUT IM KEEPING EVERYTHING CROSSED. PRESENTS WITH SIGNS AND SYMTOMS VERY MUCH WITH CERVICAL SPONDYLOTIC MYELOPATHY, SECONDRY TO SPINAL CORD COMPRESSION AT C5- C6 LEVEL. SHE HAD AN MRI SCANS AND CT SCANS THAT BOTH CONFIRMED THAT THE SPINAL CORD COMPRESSION IS SECONDARY TO MASSIVE OSTEOPHYTIC BAR FROM THE C5 VERTEBRA WITH A HUGE ANTERIOR BRIDGING OSTEOPHYTE AS WELL AT THE SAME LEVEL! NORMALLY IN AN OTHERWISE FIT AND HEALTHY INDIVIDUAL, SURGERY WOULD DEFINITELY BE THE BEST OPTION TO STOP FURTHER DETERIORATION OF THE UPPER AND LOWER LIMB FUNCTION. IN HER, HOWEVER, SHE SEEMS TO HAVE SIGNIFICANT CO MORBIDITY IN OUR UNIT,, WE HAVE AGREED TO START LMWH AS A PHROPHYLACIT DOSE 48 HRS AFTER SURGERY, PARTICULARLY IN EITHER ANTERIOR OR POSTERIOR SPINAL SURGERY, AS ANY HAEMATOMA WOULD EITHER LEAD TO SIGNIFICANT DEGREE OF SPINAL CORD COMPROMISE AND COMPLETE QUADRIPLEGIA OR SIGNIFICANT DIFFICULTY SWALLOWING AND COMPROMISE WITH BRUISING REQUIRING FURTHER REVISION SURGERY, WHICH WOULD BE ATTENDED WITH HIGH MORBIDITIES THAT MAY PRECLUDE HER FROM SURGICAL INTERVENTION. I HAVE MY DOUBTS ABOUT HER TOLLERATING GENERAL ANAESTHESIA. AT A PUSH, AND IF ALL WAS WELL AT THE TIME OF SURGERY IN TERMS OF HAEMOSTASIS, I WOULD BE HAPPY TO START ENEXOPARIN 24 HRS FOLLOWING SURGERY, AS LONG AS CATHERINE FULLY UNDERSTANDS THAT THERE IS A COMPROMISE AND HENCE, THERE IS A RISK OF BLOOD CLOT AT THE OPERATION SITE. I FULLY APPRECIATE THAT WE ARE IN A TERRIBLE SITUATION, AND IT COULD BE A CASE OF THAT FOR CATHERINE'S SAFETY, THAT WE MAY AGREE TO TREAT HER CONSERVATIVELY. I AM REALLY SORRY THIS IS SOOO LONG WINDED, BUT I HAVE TYPED MOST OF THE LAST LETTER. I GO AT 10.00AM TO SOUTHAMPTON, ON WEDNESDAY, IF THERE IS ANY CHANCE AT ALL OF ME HAVING ANY OF YOUR THOUGHTS AND/ OR YOUR OPINION I WOULD BE MASSIVELY GRATEFUL. BEST WISHES, AND BLESSINGS. CATHERINE ( KATE) PHONE IS 01202 625587 OR MOBILE 07980 916626. E MAIL IS. kate.coombs@gmail.com. 😊 ❤

Good Day... I've watched many of your videos initially because I just love medical science and your presentations are always really well done. Now, I have my own cervical spine problem and need to think about when and who I want to fix it. If I were on the west coast I'd be calling you. But, I'm in Pennsylvania. I have seen one neurosurgeon who is conservative and like you suggest, considers surgery as that last resort. He told me I'd probably need surgery in the next year or so but that he would wait a whole lot longer based upon my symptoms and imaging. I realize that ACDF is the "gold standard" for cervical stenosis with radiculopathy and/or myelopathy. However, because I've been told I need a 4 level ACDF I have investigated some other less invasive procedures that I'm wondering about. While I've seen several options, including Discseel and the like, one that interests me most is the procedure done at Deuk Spine Institute using a laser. It is basically a laparoscopic procedure that does not include fusion. I guess not having fusion is the part that makes me so interested. I'm wondering if you could either point me to any longitudinal data regarding success of that procedure and/or your own thoughts about it's success. Thanks again for your well made videos.

The first time i had it done, it was by a radiologist. I asked for something to relax.me and he told me i didnt need it. He stuck me in three different places. When he did the first one, i told him he was a liar cause it hurt like hell. It lasted a week. I went back to my ortho doctor and told him him he sent to a quack. I the. Got referred to pain management. Wish i had been sent there first. Now i have ptsd from that first guy. Pain mgmt team makes sure i get sedation now cause i start shaking as soon as i get on the table for the procedure. I have had only one bad bad experience. They decided to do a nerve ablation but you have to do two test runs first. The first one caused the nerve in my leg ro hurt all the way down to my big toe. I thought it was a blood clot and so did the trage nurse. Ended up in the er. They shot me up with something good and sent me home after testing to ensure it was not a blood clot. Didnt happen the second time. Got the nerve ablation 6 months ago and no sciatica pain since then. Knock on wood. Still lower back pain but a got trigger point in bilateral sacral joints and now absolutely pain free. I can again stand in one spot for more than 3 minutes without running for a tramadol. Love my pain mgmt team. Dont want surgery.

I experienced this (cauda equina) 14 years ago. I had already had a discectomy 1.5 years earlier. I was playing basketball and a guy landed on me, and by the end of the day I could not walk and my legs were having spasms. I had other issues too (loss of bladder control) and ended up in the emergency room having another discectomy. I was immediately better. They said I was lucky to get in when I did. And you're right, when I mentioned bladder issues they got me right in. 14 years later I've had two back fusions (neck and toe fusion too), and am actually doing really well. Your video is spot on with my cauda equina experience.

I always knew that arthritis and related stuff was aggressive in my family, but this was crazy. The sciatica in my left leg went from zero to 8 in less than a week, and it hasn't backed down from there. They want me to go into physical therapy, but this may be the only way I can do that. Now also through out the day aching pain across the sacrum area switching to stabbing pain to left butt cheek switching to pain shooting down inner left thigh to knee switching to sharp pain in groin area. I'm 70 female have complain to doctor for several years as pain keeps getting worse. With his guessing attempts I have had colonoscopy, 3 internal hemorrids removed, been to physical therapy (no help) shot in SI joint (no help) pelvic doctor says my coccyx bone bent all the way up. Pushed pushed for MRI- MANY things such as multilevel degenerative spondylosis most prominent L3-4. L5-S1 disc protrusion , severe right neural foraminal narrowing, right and left degenerative facet change ligamentum flavum hypertrophy. L4-L5 disc bulge with overlying osteophytes, effacement of anterior thecal sac L3-L4 spondylolisthesis, osteophytes, severe left neural forminal narrowing, compression of left L4 nerve root L2-L3 severe left L2 neural forminal norrowing degenerative facet changes L1-L2 disc bulge extrusion extending cephalad, anterior thecal sac, degenerative facet, ligament flavum hypertrophy , foraminal norrowing , T12-L1 disc bulge degenerative facet, ligamentum hypertrophy lateral recess narrowing The MRi was only of lumbar area though I ask to include down to coccyx. Spine dr - PA suggested epidural (did not help) sent copy of MRI to Texas Spine in Dallas, Tx mentioning concern CES have not visited I live 200 mi away Many dr there. A doctor replied don't think you have CES but referred to another dr that maybe my fecal trouble is due to coccyx bent(which does show on MRI) and other pain may be due to 19% scolocsis curve which is not even mentioned in MRI notes. I am in rotating pain ALL DAY LONG What type of surgery is done for CES and how invasive is it.

Just don’t expect a fusion to save you, bcuz it doesn’t always and it can go wrong.

Wow thanks for the great explanation…I’m dealing with a laundry list of back issues & don’t have insurance. Buzzwords are my friends!

Thanks for the very thorough explanation!! The surgeon I consulted with didn't provide any helpful info.

I'm going in later today for this. After reading some material and watching this I feel all the more comfortable. The comments were a big help as well. In conversation with others --they all seem to have had a good experience. Only negative I've heard really is from ppl afraid to have the procedure.

I had severe retrololisthesis, which I don’t hear much about, is that rare?

It's not rare it's because it usually doesn't cause a problem
